Shot Records

In order for students to attend preschool, an updated shot record must be provided by the parents.  All 3 and 4 year olds attending GCT Preschool are required by Arkansas Law to have the following immunizations:

(3 year olds): 4 doses of DTP

                     3 doses of Polio

                     1-2 doses of MMR (Measles/Mumps/Rubella)

                     3 doses of Hepatitis B

                     3 doses of HIB

                     1 dose of Varicella (chickenpox vaccine) or a document provided by the school and

                     signed by the parent, stating the student has had the disease

(4 year olds): 5 doses of DTP (last dose must be after child's 4th birthday)

                     4 doses of Polio (last dose must be after child's 4th birthday)

                     1-2 doses of MMR (Measles/Mumps/Rubella)

                     3 doses of Hepatitis B

                     3-4 doses of HIB

                     1 dose of Varicella (chickenpox vaccine) or a document provided by the school and

                     signed by the parent, stating the student has had the disease
